Sports Physical Clinics

Logan Health doesn’t want you on the sidelines this season. All Montana student-athletes are required to receive a sports physical each year in order to participate in all school sports and many club sports. Getting an annual sports physical helps protect children from worsening injuries or illnesses during athletic activities. We recommend that you schedule sports physical appointments after May 1, so that it remains valid throughout the following school year. 

Sports physicals are always available through your child’s primary care provider, or during normal appointments at the school-based health centers. Students will either need to be accompanied by a parent or guardian, or must bring a completed sports physical form signed by their parent or guardian to the appointment. 

2023-2024 School Year Physicals

This Spring the Logan Health School-Based Health Center and Youth Development programs are holding sports physicals for all athletes around the valley on June 12th from 9:00AM to 6:00PM hosted by Flathead Field at Glacier Bank Park.  

**Please call (406) 858-3454 to make an appointment.  Walk-ins are allowed and will be fit in when there is availability.
**Cost is $35.  Bring cash or check made payable to Logan Health with you.  

***Please scroll down and download the three forms at the bottom of this page.  Fill them all out and bring them with you to your physical.  Parents do not need to be present for the physical but ALL forms must be SIGNED BY PARENT for us to be allowed to complete the full physical exam.
