The DMCA specifies that all claims must include the following information:
- A physical signature of the copyright owner or the person authorized to act on behalf of the owner;

- Identification of the material that is claimed to be infringing and information reasonably sufficient to enable Logan Health to locate the alleged infringement on the Logan Health website;
