How It Works

  1. Speak with a nurse who screens your symptoms.
Tell us about yourself and your reason for visiting. We will make sure you can use our virtual service.
  2. Submit a virtual questionnaire.
The virtual questionnaire is a standard intake form that creates your virtual visit.
  3. Begin the visit and tell the provider your concerns.
Talk about the reason for your video visit with a provider.
  4. Get a treatment plan.
Your provider will address your concerns and give you treatment options. Depending on the reason for the visit, your provider may recommend additional visits in person.

Cost and Insurance

  • The cost of your visit will depend on the level of care provided. An estimate is $250, but the visit may be more or less.

Home-Based Virtual Care
