Keep Your Athlete in the Game
At Logan Health, we want Montana student-athletes to stay safe and active. All student-athletes are required to receive an annual sports physical to participate in school and many club sports. These exams help prevent injuries or illnesses from worsening during athletic activities and ensure students are ready to perform safely.
Scheduling Recommendations:
- Schedule sports physicals after May 1 to remain valid for the entire upcoming school year
- Available through your child’s primary care provider or at school-based health centers during normal appointment hours
- Students must be accompanied by a parent or guardian, or bring a completed sports physical form signed by a parent/guardian
