Logan Health doesn’t want you on the sidelines this season. All Montana student-athletes are required to receive a sports physical each year in order to participate in all school sports and many club sports. Getting an annual sports physical helps protect children from worsening injuries or illnesses during athletic activities. We recommend that you schedule sports physical appointments after May 1, so that it remains valid throughout the following school year. Sports physicals are always available through your child’s primary care provider, or during normal appointments at the school-based health centers. Students will either need to be accompanied by a parent or guardian, or must bring a completed sports physical form signed by their parent or guardian to the appointment.

July 29 – Sports Physicals Clinic in Sunburst, MT
Logan Health is hosting a sports physicals clinic open to all school-aged kids on Tuesday, July 29 from 10 a.m. – 1 p.m. at North Toole County High School auditorium.
Cost is $20 cash or check made payable to Logan Health.
Walk-ins welcome, appointments encouraged to reduce wait times. Call (406) 966-7040 to schedule an appointment and ask for “Sunburst Sports Physical Day”. Students will either need to be accompanied by a parent or legal guardian, or must bring a completed sports physical form signed by their parent or guardian to the appointment.

Students will either need to be accompanied by a parent or legal guardian, or must bring a completed sports physical form signed by their parent or guardian.
August 6 – Walk-in Sports Physicals Clinic in Cut Bank, MT
Logan Health is hosting a sports physicals clinic open to all school-aged kids on Wednesday, August 6, from 8 a.m.. – 3:30 p.m. at the Cut Bank High School Library.
This event is provided at no cost.
Students will need to be accompanied by a parent or legal guardian and must bring completed forms.

Students will either need to be accompanied by a parent or legal guardian, or must bring a completed sports physical form signed by their parent or guardian. The Health Department and Blue Cross Blue Shield care van will be on site for immunizations as well.
