Logan Health – Whitefish does not exclude, deny benefits to, or otherwise discriminate against any person on the grounds of race, color or national origin, or on the basis of disability or age in admission to, participation in, or receipt of the services and benefits under any of its programs and activities, whether carried out by Logan Health – Whitefish directly or through a contractor or other entity.

If you believe that Logan Health – Whitefish has failed to provide these services or discriminated in another way on the basis of race, color, national origin, age, disability, or sex, you can file a grievance with the Logan Health – Whitefish Quality Department, 1600 Hospital Way, Whitefish, 59937, (406) 863-3500 or quality@nvhosp.org. If you need help filing a grievance, the quality department staff are available to help you.

Notice of Program Accessibility
Logan Health – Whitefish and its entities are accessible to and usable by disabled persons, including persons who are deaf, hard of hearing, blind or who have other sensory impairments. Access features include assistive and communication aids at no additional charge for such aids. These aids include:
  • Qualified sign language interpreters
  • Communication boards
  • TTY Telephone
 
If you require any of these aids, please let the receptionist or your nurse know.
