Home Options
Home Options provides services using the Medicare Care Choices Model, a new option that allows patients to receive in-home services while still receiving care from their primary provider. The goal of Choices is to improve quality of care and increase patient and family satisfaction.

  • Nursing care for symptom management, disease education and disease management, and care during all phases of chronic disease
  • Medication teaching and management
  • Social services for advance directives and end-of-life planning
  • Spiritual care
  • Aide and homemaker services
  • Volunteers for support and in-home respite
  • Nutritional support
  • Bereavement services
Choices services are available 24/7.

Choices Eligibility

The Centers for Medicare and Medicaid Services has revised the eligibility for Choices:
  • Dual eligible Medicare and Medicaid patients may now qualify for Home Options Choices, a group of in-home services provided by Home Options.
  • Home Options Choices provides selective services while patients still receive curative, palliative or aggressive treatment.
  • The primary provider continues to manage the patient’s treatment.
  • Participants must meet specific criteria, in addition to having one of the following diagnoses: congestive heart failure, chronic obstructive pulmonary disease, advanced cancers or human immunodeficiency virus.
  • The patient may or may not be receiving curative treatment.

After receiving a referral to Choices, Home Options staff review each patient’s record to ensure they meet the criteria. The referring provider must complete the Home Options Choices attestation that the patient’s life expectancy is six months or less if the disease should run its normal course.
