The goal of chronic care management is to improve health care quality for patients living with chronic illness. Chronic care management includes monthly clinical staff time to provide support and coordination of services, monitoring, education and self-management tools.
Who qualifies for chronic care management?
Individuals who have two or more chronic health conditions are eligible for care management.
How does it work?
Once enrolled in chronic care management, you will receive ongoing monthly attention under the direction of your provider until you choose to stop. There may be a copay with your insurance. You may opt out at any time.

We currently offer a laboratory that is certified by the College of American Pathologists and Clinical Laboratory Improvement Amendments, the highest regarded laboratory recognition in the country. Laboratory specimens ordered through your provider at this clinic are collected daily in Columbia Falls and processed through the laboratory at Logan Health Whitefish.

This program is designed to provide essential medical services regardless of the patient’s ability to pay. Discounts are offered based upon household income and size. A sliding fee schedule is used to calculate the basic discount and is updated each year using the federal poverty guidelines.
