Onboarding Requirements
1. TB – Tuberculosis Testing/Immunity:
*Proof of 1, 2 or 3 required.

- Negative Quantiferon or T-SPOT TB blood test in the last 12 months.
- Record of two negative TB skin tests (two-step) in the last 12 months. If you are providing the two-step documentation the second test must be initiated between 7-21 days after the first negative test was read.
- Record of negative QFT or T-SPOT or two-step TB skin test dated more than 12 months ago PLUS documented proof of annual TB surveillance form and or annual TB education.*If you are a positive responder, please contact Logan Health Student Affiliations for the required documentation.
2. Negative Background Check:
*Must include the following:

- National Criminal Background
- National Sexual Offender Registry
- Federal General Services Administration and Federal Health and Human Services Office of Inspector General (OIG)
3. MMR (measles, mumps, rubella)
*You are not required to provide proof of vaccination but if you do not do so, we will consider you to be unvaccinated.
Provide one of the following:
- 2 official records receiving vaccinations
- Positive titer results for each measles, mumps and rubella proving immunity
4. Varicella (chickenpox)
*You are not required to provide proof of vaccination but if you do not do so, we will consider you to be unvaccinated.
Provide one of the following:
- Record of 2 vaccinations
- Positive titer results proving immunity
5. Hepatitis B
*You are not required to provide proof of vaccination but if you do not do so, we will consider you to be unvaccinated.

- Provide proof of vaccination – Titer with reactive or positive results.
6. Tetanus-Diphtheria-Pertussis (Tdap) –
*You are not required to provide proof of vaccination but if you do not do so, we will consider you to be unvaccinated.
Provide proof of vaccination – One dose of Tdap (Tdap only – does not include DTaP or Td or tetanus)
Injunction was lifted on Montana House Bill 702. In response to this change, the following changes are occurring at Logan Health.
Do I need to be vaccinated to be a student or instructor at Logan Health?
Vaccines are not required.
What happens if I am not vaccinated or decline to provide proof of vaccination?
A representative from Logan Health will contact you via the phone number you provide to have an individualized conversation.
Are the vaccine recommendations different for employees and students/instructors?
No, the vaccine recommendations are the same for Logan Health employees and students/instructors.
Is Tuberculosis (TB) testing still required?
Yes, TB testing is still required.
Placement of Out-of-State Nursing Students
Montana ARM 24.159.608:
Any student in nursing school who is not already a registered nurse (this could also be anyone in an LPN to RN program) and is attending an out-of-state nursing school, must obtain permission from the Montana Board of Licensing for permission to do clinical hours in the state of Montana.
