Chester Patient Portals
PatientConnect
Starting April 1, 2025, Logan Health – Chester is going live with a new electronic health record through Oracle. Patients can access medical health records through the new patient portal called PatientConnect.
PatientConnect allows you to easily access your electronic medical records, as well as pay your bill online.
Who can sign up for PatientConnect?
All patients aged 18 and older. A legal guardian may access the adolescent’s record with proper authorization.
How to register for PatientConnect Medical Record Portal
To register online, you will need your Medical Record Number (MRN). You can find your medical record number in several locations:
- On a medication list or any discharge paperwork you may have received at an appointment with your provider.
- On any medical record you may have.
With PatientConnect, you can:
- View and print your lab results, allergy list, immunizations records, medication list, health issues, surgeries and procedures
- View and print your medical notes from your health care provider (for visits on/after 2/1/2025)
- View your upcoming appointments
- Communicate with your doctor’s office by sending secure messages
- Request a prescription renewal through the secure message tool
myHealth
To view medical records from Logan Health – Chester from December 1, 2023 – March 31, 2025, patients should log into their myHealth patient portal account.
myHealth is a free, secure patient portal that provides the most accurate, up-to-date information available to you about your care at Logan Health, and it connects you to your health care team. Using the myHealth patient portal, you can take a more active role in your care in an easy, secure way. Learn more about myhealth here.
