Medical Fitness Center Vacation Freeze Form

The Vacation Freeze will temporarily suspend your account for up to two months for $25/month, regardless of membership type or size. At the end of a Vacation Freeze, billing will resume automatically.

Please complete this form at least three business days before the end of the month to avoid full membership dues for the following month.

Vacation Freeze Details:

• A Vacation Freeze must begin on the first day of the month and goes through the last day of the month.
• I understand that to initiate a Vacation Freeze, Member Services must be notified three business days prior to the month of the requested freeze. If I do not give the required notice, I understand that additional fees may be assessed.
• During a freeze, account members cannot access Logan Health Medical Fitness Center using their Logan Health membership privileges, nor do they have access to their account guest passes.
• Maximum of two months each calendar year at a flat rate of $25/month.
• If I paid a Year In Advance (YIA) I will be charged $25/month at the time of request, and my YIA payment will be extended one or two month(s).
