Logan Health Transitions of Care provides short-term follow-up care for patients after a hospital or emergency department visit or during gaps in primary care coverage. The service helps bridge care transitions by offering post-hospital follow-up, transitional care management and short-term medication refill support until long-term primary care is established.
Located at 1287 Burns Way in Kalispell, Logan Health Transitions of Care is open Monday through Friday from 8 a.m. to 5 p.m. Patients should use the elevator to access the basement level.
