Columbia Falls School-Based Clinic
Launched in 2016, the North Valley School-Based Clinic
is provided through collaboration between North Valley Hospital, North Valley Behavioral Health, North Valley Family Medicine and the Columbia Falls School District Six.
North Valley Family Medicine and North Valley Behavioral Health medical staff provide Columbia Falls High School students and staff with medical, wellness, health education and behavioral health services on-site at the high school with the goals of improving the health of students and staff, increasing access to health care and decreased time lost from school.

To help you and your family navigate the complex world of health care, we can offer assistance and motivation to reach your health goals.
The goal of chronic care management is to improve health care quality for patients living with chronic illness. Chronic care management includes monthly clinical staff time to provide support and coordination of services, monitoring, education and self-management tools.
Who qualifies for chronic care management?
Individuals who have two or more chronic health conditions are eligible for care management.
How does it work?
Once enrolled in chronic care management, you will receive ongoing monthly attention under the direction of your provider until you choose to stop. There may be a copay with your insurance. You may opt out at any time.

The Laboratory at NV Family Medicine in Columbia Falls
At NV Family Medicine, North Valley Hospital currently offers radiology (X-ray and ultrasound) services and operates a laboratory that is certified by the College of American Pathologists and Clinical Laboratory Improvement Amendments, the highest regarded laboratory recognition in the country. Regardless of where your doctor or health care provider is located, you can have your laboratory work done at this location. The specimens are collected daily in Columbia Falls and processed through the laboratory at North Valley Hospital.

Sliding Fee Discount Program
North Valley Family Medicine offers a sliding fee discount program to all those in need, whether or not they have insurance. This program is designed to provide essential medical services regardless of the patient’s ability to pay. Discounts are offered based upon household income and size. A sliding fee schedule is used to calculate the basic discount and is updated each year using the federal poverty guidelines.
