Student and Instructor Parking Map

All students and instructors are required to park in the student parking area designated on the map below. Failure to do so can lead to termination of the student experience here at Logan Health. All students and instructors are permitted to ride the Logan Health campus employee shuttle at no charge when it is in operation.

Documents and Policies to Review

Please review the following documents that include the requirements for doing an educational rotation here at Logan Health.

1d. Review GNNC780 Nursing Student Clinical Rotation Policy
2. Fill out the Instructor Personal Information Form. This will need to be filled out prior to the beginning of any and every semester, quarter or rotation. This form will be sent directly to the Logan Health Student Coordinator when you hit the submit button.

3. Fill out the Background Check Release Form and submit it to your school contact or submit it directly to the Logan Health Student Coordinator.
Please check with your school contact to verify your responsibilities to avoid delays in starting your experience with Logan Health.

Computer Access for Instructors

Please fill out the following two linked documents if you will need computer access for your rotation. These documents must be filled out and emailed or faxed to the Student Coordinator at least 30 days prior to starting the semester or quarter.
