Logan Health doesn't want you on the sidelines this season. All Montana student-athletes are required to receive a sports physical each year in order to participate in all school sports and many club sports. Getting an annual sports physical helps protect children from worsening injuries or illnesses during athletic activities. We recommend that you schedule sports physical appointments after May 1, so that it remains valid throughout the following school year.
Sports physicals are always available through your child's primary care provider, or during normal appointments at the school-based health centers. Students will either need to be accompanied by a parent or guardian, or must bring a completed sports physical form signed by their parent or guardian to the appointment.
Spring 2022 Clinics
This Spring the Logan Health School-Based Health Center program is hosting sports physicals around the valley. Student's receiving sports physicals will be charged $35. Logan Health will donate a portion of the proceeds to the school to benefit their activities and athletics programs.
